Student Recommendation Form
High School counselors need to complete a recommendation form for each student applying to Discovery Academy. The completed recommendation form should accompany each application when sent in to the office of Academic & Innovative Partnerships at St. Cloud Technical and Community College.
Request for Transcript form
This form can be used by current Discovery Academy students who would like an official copy of their St. Cloud Technical and Community College transcript.
To request a transcript:
- Print out Transcript Request Form
- Submit the form to the Records and Registration Office by:
- Hand delivery
- Mail to: Records and Registration Office

- Include $7 fee for each official transcript requested.
Transcripts are run at the end of each business day and mailed out.
Turnaround time is generally 24 hours from the time the request is received.

Field Trip Permission Slip
Students enrolled in Discovery Academy have the opportunity to explore local industries within their field of study through occasional field trips. A field trip permission slip can be downloaded here as needed. Discovery Academy instructors will also provide these in the classroom.
Credit Equivalency
Students who successfully complete their coursework in Discovery Academy receive transcripted college credit at St. Cloud Technical and Community College. This table shows how the courses offered through Discovery Academy directly transfer into St. Cloud Technical and Community College, and in which majors the credits apply.
